Telehealth Consent
What a virtual visit covers — and what it doesn't.
What telehealth means here
Telehealth is care delivered without you and your clinician being in the same room. In this practice that includes intake forms, secure messaging, phone calls, video visits, lab review and follow-up check-ins.
Your clinician uses the history you provide, your lab results and your reported symptoms to build a plan. Where a hands-on exam or in-person testing is required, you will be referred locally.
Benefits
Care without travel or waiting rooms, faster follow-ups when a dose needs adjusting, and direct access to the clinician managing your plan rather than a rotating panel.
Limits and risks you should understand
Without a physical exam, some conditions can be missed or misjudged. Information you provide may be incomplete, and lab work may not capture everything relevant.
Technology can fail. A dropped call, an undelivered message or a delayed lab result can postpone care. Electronic transmission, while protected, is never entirely without risk of interception.
In rare cases your clinician may determine telehealth is not appropriate for your situation and will decline to treat virtually, referring you to in-person care instead.
Prescriptions
A consultation does not guarantee a prescription. Medications — including GLP-1 therapies, hormone replacement, and performance and intimacy medications — are prescribed only when a licensed clinician determines they are clinically appropriate and reasonably safe for you.
Prescriptions are sent to a licensed pharmacy. Compounded formulas are prepared per prescription and are generally not returnable.
You may be asked to complete lab work before a prescription is issued, and periodically thereafter to continue it.
Licensure and location
Care can be provided only where the clinician holds an active license. You must tell us the state you are physically located in at the time of each visit, and update us if that changes.
If you are outside the states this practice is licensed to serve, we will tell you before charging for a consultation and will help you find local care.
Your rights
You may ask questions at any point, request a copy of your records, ask for a different clinician where one is available, or decline any part of a recommended plan.
You may withdraw this consent at any time by writing to jasmine4med@yahoo.com. Withdrawing consent stops future telehealth care; it does not undo care already provided or erase records we are required to keep.
Records, billing and confidentiality
Visit notes, lab results and messages become part of your medical record and are handled under our Notice of Privacy Practices.
Consultation fees are disclosed before you are charged. Telehealth visits and compounded therapies are generally not billed to insurance; you may request an itemized receipt to submit yourself.
Your acknowledgement
By completing an intake and attending a consultation, you acknowledge that you have read this consent, that you have had the chance to ask questions, and that you consent to receive care by telehealth.
